To order via our website you must register for this service. Please complete and return the Application Form below or contact reception for a paper copy.
For items that are not on your repeat list please complete the Special Request form within the Acute Prescriptions tab.
You may obtain a repeat prescription by completing your re-order form and placing in the box at the surgery; posting or ordering via our website before 12 noon on a weekday, for receipt in 48 hours. Please note we do not accept medication requests by telephone in the interests of patient safety.
We now have a manned dedicated telephone prescription line operating between the hours of 10 am and 3 pm. You should use this number if you do not wish to submit your requests online. You can discuss any queries you have regarding your medication with the administrator and order your medication.
PLEASE DO NOT ATTEND TO COLLECT YOUR PRESCRIPTION AT THE PHARMACY OR SURGERY BEFORE ALLOWING 2 FULL WORKING DAYS TO PASS(NOT INCLUDING WEEKENDS AND PUBLIC HOLIDAYS).
All patients on repeat prescriptions must see the doctor at regular intervals.
If you require medication no longer on your repeat list, please complete the form below and either hand in to the surgery or email the form to us at FV-UHB.firstname.lastname@example.org. Please note this means you will have to open and save the form on your own computer in order to send it as an attachment to our email address.
Future development of this page will allow the form to be sent automatically on completion but, for the moment, we can only receive the form printed and handed into the surgery or via email.
Special Request order form
The special request order form has been created to improve patient safety. Recent national surveys indicated there is a high probability of prescription errors when information is taken over the telephone. For this reason, prescription requests should not be made via telephone.
Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery.
